1.       Review of all the procedures and principles with all your heart of NCM 101 skills this summer.

2.       Assembly time for BSN104,

a.       Assembly place: Pavillion

b.      Assembly time: 10:00am

c.       Holding area: LB101

d.      Revalida instructor: Ms. Monica Anonuevo, Level III coordinator

e.      Color of band: orange

3.       No loitering within the vicinity of FEU while activity is being held

4.       Complete RLE uniform

a.       Male- Well-shaved, 2x3 haircut, no side burn

b.      Female- hair tied and bun, colored undergarments are not allowed

5.       Student is required to wear identifying bands on their right arm, NO BAND, NO ENTRY

6.       The traffic team will only allow required materials to be brought in, strict checking will be done on the entrance

7.       Only allowed and required materials are CHN bag and its materials inside (Refer to the list I’ve given you to buy)

8.       FEU will no be responsible for any losses inside the vicinity

9.       Absents and tardiness are not tolerated; holding rooms closed 15 minutes before testing time

10.   Late comers: I.D. is confiscated and deduction of 10% of your total score in revalida. ID could be claim with an explanation letter.

11.   All nursing procedures during summer are included, e-tool would be distributed at the holding room

12.   Students are given 15 minutes, randomly assigned, answer questions and accomplish completely

13.   Examiners are required to show score immediately, sign at the grade given, don’t sign if the grade is not indicated

14.   If you wanted to go the CR, surrender your id and use only the current floor

15.   After you finish your revalida, leave FEU immediately to gate 2 (Gate where LRT2 is based), if not leave immediately, they will be sanctioned to -10% of total score in revalida

16.   No make up exams to those who will miss it
